Documentation that Verifies Eligibility to Work in the United States and Identity

We only hire workers who are legally documented to work in the U.S. Upon hire, applicants must provide UniSea with documentation that verifies eligibility to work in the United States and their identity. For a complete list of acceptable documents, please refer to Form I-9. You must furnish valid original documents or certified copies.

You must provide ONE document from LIST A:

LIST A

  • U.S. Passport - Certificate of U.S. Citizenship (INS Form N-560 or N-561)
  • Certificate of Naturalization (INS Form N-550 or N-570)
  • Unexpired Foreign Passport with I-155 stamp or attached INS Form I-94 indicating valid employment authorization
  • INS Form I-151 or I-551 (Alien registration receipt card with photo)
  • Unexpired Temporary Resident Card (INS Form I-688)
  • Unexpired Employment Authorization Card (INS Form I-688A)
  • Unexpired Reentry Permit (INS Form I-327)
  • Unexpired Refugee Travel Document (INS Form I-571)
  • Unexpired Employment Authorization Document (INS Form I-688B, contains photo)

You must provide TWO DOCUMENTS 
(one from LIST B and one from LIST C):

LIST B
  • Driver’s license or ID issued by federal, state or local agencies containing photo, or vital statistics, such as address, name, date of birth, sex, height and eye color.
  • U.S. Military card or draft record
  • Military Dependant ID Card
  • Native American Tribal Document
  • U.S. Coast Guard Merchant Mariner Card
  • Driver’s License issued by Canadian government authority
  • School ID with photo
 
LIST C
  •  U.S. Social Security Card (cannot state it is “Not Valid for Employment”)
  • Certification of Birth Abroad issued by the Dept of State (Form FS-545 or DS-1350)
  • Voters Registration Card
  • Original or certified copy of Birth Certificate issued by state, county, municipal authority or outlying possession of United States bearing official seal
  • U.S. Citizen ID Card (INS Form I-197)
  • ID Card for use of Resident Citizen in the United States (INS Form I-179)
  • Unexpired employment authorization document issued by INS not listed in list A.
